This story does not make sense - for how long did your "cuz" have this phone ? When did it start asking for a lock code when switching on ? What exactly does the screen say ?

Try the standard 12345 for starters. If its different from that, then someone has changed it (someone must have activated it anyway), and if it wasnt him, its not an easy unlock.

I still cannot understand how he managed to forget it between changing it and the next time he turned it on.

Take it to phone stall where they unlock/reflash/etc. mobile phones and they should be able to sort it out with the gear they have. Clearly it will not be for free.
